Create a digital form

This guide explains how to design a form that will be completed by your Clients and whose results will automatically appear in your Coach Space .

Step 1: Access the creation tool

1. In the Cognito Coach navigation menu, click the Forms tab.
2. Click the Create button.

Step 2: Provide basic information

A new window (or page) will open for initial setup. Fill in the following fields:

1. Form Title : Give your form a clear and explicit name (e.g., Meeting Preparation , Eating Habits Survey , etc.).
2. Form Details : Add a brief introduction or instructions for your customers. You can include text, links, or even embed a video.

Step 3: Add the questions

Once the basic setup is complete, you can start building your questionnaire.

Question Write your question clearly.
Response type Choose the desired response format (the system automatically adapts the entry for your Customers.
Text : For open answers (short or long).
Number : For encrypted data only.
Single choice : Yes/No
Choice from a list : Multiple choice, create each option from the list.
Indicate whether the question is mandatory : To be completed or not by checking the box.

If you would like to add a section

Add a section: To add a section, in the question text, before or after your question, indicate the section and its number and then add spaces with your question. This makes the form less intimidating and easier to complete.

Step 4: Edit and Manage

To edit an existing form or template:

• Return to the Forms tab.
• Click on the desired form in the list.
• Edit the text.
